4. Through the Request-A-Form Service. Call 1800-11-00-00 (toll free), 0120-2445252 or log onto www.amity.edu/admission and give your details. A form will be couriered to you free of cost. The form cost of Rs. 1,100/- in cash will have to be paid to the courier man.
