Eligibility: The candidates must have completed the age of 17 (Seventeen) year and be within 28 years of age as on 30.06.2005. For S.C/S.T. candidates upper age limit shall be relaxed by 5 (Five) years. The candidates must have passed I.Sc. or +2 Science from any University/Council of Higher Secondary Education, Orissa or any other Examination recognized equivalent thereto.

Medical College & Hospital, Burla-768017 on payment of Rs. 200/- through Bank Draft (A/C Payee) only drawn in the branch of State Bank of India situated at the same place i.e. State Bank of India. S.C.B.M.C Campus Branch, Cuttack/State Bank of India, Burla Branch, Burla payable to the Principal Tutor, School of Nursing from whom the Prospectus and Forms are desired to be obtained with an application in plain paper and self addressed envelope (size 23 cm x 10 cm) affixing postage stamp worth of Rs. 32/- only for dispatch by Registered Post or Rs. 10/- only in ordinary post or by submitting the said Bank Draft along with application on plain paper for the forms to be obtained personally. The senders Full Name and complete postal address should be written in Block Letters on the envelope attached to the application and on the reverse of the bank Draft. The words Application Form for General Nursing and Midwifery Training Course should be written prominently in Block Letter on the body of the envelope.
